   1.  Print the completed form and mail to The Montessori Academy at Edison Lakes, 530 East Day Road, Mishawaka, IN 46545.
   2.  Fax the printed form to 574.256.5493.
   3.   If you would like to email this completed PDF and the Submit button does not work in your email program, please follow these steps:
        •  In your internet browser toolbar, click on FILE, SAVE AS, and save the PDF to your desktop
        •  Open an email using your email provider and attach the completed PDF to the email
        •  Email it to jgabrielse@tma-el.org
